Joint Request for Mediation

Mediation is a process whereby a Neutral attempts to reason with the parties to facilitate a voluntary settlement by focusing attention upon the parties needs and interests rather than upon their rights and positions.

Parties may file a joint request using the forms below and submitting the non-refundable $500 ($600 for non-CAI members) filing fee.

Joint Request/Agreement for Mediation Form

Step 1:      Initiator and Respondent must complete and return with fee the joint request/agreement for Mediation Form

 
Initiator's Statement of Dispute  Respondent's Statement of Dispute   

Step 2:     Initiator and Respondent must complete and sign the statement of dispute and identify all parties involved in the dispute.

Step 3:     Either party may be represented by counsel but must give advance notice to all parties if they elect to be represented.
